A RESOLUTION AUTHORIZING THE CITY OF JACKSONVILLE'S
PURCHASING AGENT TO PROCESS AND INITIATE CLAIM FORMS
FOR PARTICIPATION IN THE STATE OF ARKANSAS WORKMEN'S
COMPENSATION INSURANCE PLAN
WHEREAS, the City Council of the City of Jacksonville,
Arkansas voted unanimously to participate in the State of
Arkansas Workmen's Compensation insurance plan;
WHEREAS, the City Council of the City of Jacksonville,
Arkansas desires that the City Purchasing Agent be responsible
for the processing of claim forms in conjunction with the
program; and
WHEREAS, there is a need for City employee to handle the
claim forms.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF
THE CITY OF JACKSON TI , ARKANSAS, that;
SECTION 1. The City Council of the City of Jacksonville does
hereby authorize that the person holding the position of City
Purchasing Agent be responsible for processing and initiating
claim forms as required for participation in the State of
Arkansas Workmen's Compensation Insurance Plan.
SECTION 2. This resolution being necessary for the preser-
vation of the public health, safety and welfare, an emergency is
hereby declared to exist and this resolution shall take effect
immediately after its passage.
